Time to Complete "Back to School" Forms
It is that time of year again for all families to complete the online “Back to School” forms. These forms include the student demographic and EMA for grades PreK-12, medical update for grades PreK-12, and transportation form for grades PreK-6. All forms must be completed before the first day of school.
 
 To complete the online forms, go to the district website at www.bridgeportschools.net, click on parent resources, and click on "Complete/Update Online Forms" on the right side of the page. Once logged in, click on "Submit and View Online Forms, update each form listed under the heading "2021-22 Back to School Forms". Once each form is completed, be sure to click on "Save and Submit to the district". If a field is not complete, a message will appear to complete the required highlighted fields and to submit again. The status will say submitted by each form when submission is successful. Make sure that you choose each child and update each child's forms. If you forget your username and password, click on "Lost Password” to retrieve your username and/or password.  The use of iPads is very important to your child’s education for grades 3-12.  By getting the online forms completed ahead of time, your child can receive their iPad sooner.
 
 Once Back to School Forms are submitted, all families must submit the family lunch application in Pay School Central. The link to login is also on the website under Pay Fees in Parent Resources.  Choose “I forgot my password” if you need assistance with your login. New parents can register for an account on this page as well.
